I worked for the California state system, starting as a Correctional Officer and retiring as a Lieutenant in 2005. I now write for the PacoVilla blog which is concerned with what could broadly be called The Correctional System.
That is a disciplinary report. It can be very minor (i.e. you were a dick, don't do it again) or very serious (we are throwing you in the hole and prosecuting you for killing your cellie) or somewhere in between. Most of course are somewhere in between. If found guilty they can result in loss of good time credit which can mean an extended stay in prison, loss of privileges and loss of programs depending on the nature and seriousness of the offense..

Never heard of one, don't know what it is, so probably not.
An arrest is public record A mug shot is public record. That being said there are now a fair number of news outlets, at least in CA, who have recently stopped the practice of publishing mug shots of people not yet convicted.
I was always somewhat formal. It worked for me, that was my personality. Some people were more towards the friendly side, others more hostile. The inmates can handle pretty much anything but vacillation If you are an asshole, be an asshole all the time. If you are officer friendly, be officer friendly all the time. MOST inmates want to do their time with a minimum of drama. They react reasonably well to being treating like human beings in awkward circumstances. That is reality, they can relate to it.
